
The Falls Prevention Program is active at Twelve Trees in order to reduce the amount of falls and in turn cut down on un-necessary hospital visits and illness that so often result after a fall within the elderly community.
Some residents are at more risk of falling than others and so it is vital that we undertake a falls risk assessment for all our residents so that we can take sufficient measures to reduce risk.
Some personal factors to consider when assessing risk are:
Vision - Eye and Vision tests
Hearing - Hearing Tests and Hearing Aid Equipment Testing
Footwear - Suitable Footwear (outside and indoors)
Blood Pressure
Hydration
Of course there are many other factors we will consider when making an assessment including; pets, environment and other personal issues and of course each assessment is bespoke because we are all individuals.
