Care

After an initial assessment to decide the best way to care for a
new resident, an individual Care Plan is completed. This document
is very detailed and has all the relevant information as to medical
and health needs, through to the time a resident would like to
have breakfast in the morning. In addition, two reports are completed three times a day, to monitor mobility, diet and general health.

All hospital visits can be escorted. This is important for reassurance and to present all the medical information to the hospital to help with care, particularly if a resident has memory problems.

Medication reviews take place every 3/4 months with our local GP practice and pharmacy. It is important to monitor and discuss medication, to ensure each resident is receiving the best possible treatment.

An active Falls Prevention program operates within the home. A resident who is identified as at risk of falling can be helped in a number of ways, including risk assessment, hip protectors, chair and bed monitors and making sure enough liquids are consumed during the day.

The District Nurse team visit twice a week to support the home and GP practice. It is a very good service and helps with samples, dressings and medical advice.

Chiropodist and Dentists are regular visitors and an important part of keeping healthy.

STAFFING

NVQ's are important qualifications and a way of demonstrating care knowledge. Over 75% of our care team are qualified in NVQ 2 / 3 & 4.

Key training takes place in the first six weeks of joining the home, including moving /handling and privacy/dignity.
